1:1 RTT Hypnotherapy & Coaching Services • Practitioner: Stacy Baumstark • Effective 2026

Welcome to Solvella LLC. This Client Agreement & Consent Form governs your participation in 1:1 Rapid Transformational Therapy (RTT®) and/or coaching services. Please read carefully and complete all sections. By signing, you confirm your full understanding and agreement.

Section A — Client Information

Collected at intake:

  • Email address
  • Phone number
  • City & state
  • How you heard about Solvella

Section B — Services & Investment

Your booking confirmation will specify the program or session type, session date(s), and total investment. Payment is due in full prior to session delivery unless otherwise agreed in writing. By signing this agreement, you confirm that payment arrangements have been confirmed.

Section C — Health & Eligibility Screening

To ensure RTT is appropriate for you, you will be asked to answer the following honestly during intake. All information is held in strict confidence.

  • C1. Are you currently under the care of a physician, psychologist, or psychiatrist?
  • C2. Are you currently taking prescription medications for mental health?
  • C3. Have you ever been diagnosed with or treated for psychosis or schizophrenia, bipolar disorder, epilepsy or seizure disorders, or active suicidal ideation? If yes, please describe.
  • C4. Primary goals for our work together.

Section D — Consent & Acknowledgments

You will be asked to initial each item to confirm agreement:

  • D1. Nature of RTT: RTT® is a non-medical, non-psychological self-development modality. It is not a substitute for medical care, psychotherapy, or mental health treatment.
  • D2. No Guarantees: Results are not guaranteed and depend on your commitment, participation, and follow-through.
  • D3. Emotional Experience: RTT may involve emotional recall, release, or temporary discomfort. You voluntarily assume responsibility for your emotional experience during and after sessions.
  • D4. Recordings: Your session may include a personalized transformation recording for your personal use only. The live session will not be recorded by the practitioner without your separate express written consent.
  • D5. Confidentiality: All sessions are confidential except where disclosure is required by law or there is risk of harm to self or others.
  • D6. Release of Liability: You release Solvella LLC, its owner, and affiliates from any and all liability arising from your participation in RTT or coaching services.
  • D7. Not a Crisis Service: Solvella LLC does not provide emergency services. In a crisis, contact 911 or the 988 Suicide & Crisis Lifeline.
  • D8. Age & Eligibility: You confirm you are 18 years of age or older and have legal capacity to enter this agreement.
  • D9. Telehealth Consent: You consent to participate in sessions via phone or video and understand the inherent privacy risks of electronic communication. You will participate from a private, secure location.
  • D10. Policies: You have read and agree to Solvella LLC's Cancellation Policy (48-hour notice required), Refund Policy, Privacy Policy, and Terms of Service as published at solvellawellness.com.

Section E — Client Commitment

You commit to showing up fully for our work together — to each session prepared, on time, and open to the process. You understand that transformation requires your active participation and follow-through between sessions, including listening to any transformation recordings as directed. You take full responsibility for your choices, actions, and results.

Section F — Governing Law & Electronic Signatures

This Agreement is governed by the laws of the State of Wisconsin. Electronic signatures — including typed name acknowledgments, checkbox confirmations, and digital signature platforms — constitute valid and legally binding signatures. Any disputes shall first be submitted to good-faith mediation before formal legal action.

Section G — Signatures

The signed agreement is executed between the Client and the Practitioner, Stacy Baumstark, RTT Practitioner & Mindset Coach, Solvella LLC. A copy will be provided to you for your records.
